We have enclosed our new employee packet for your review, completion and signature. Once this is completed, I look forward to meeting you and reviewing this information:

• current resume
• negative drug screen
• Copies of your NBRC credentials
• A diploma or certificate of completion from your program
• An Indiana RCP state license
• Copies of your MMR vaccines or titers
• An up to date PPD (TB) test or interpretation of an X-ray
• A copy of Hepatitis B vaccination or declination
• A current BLS Healthcare Provider (CPR) and/or ACLS card
• A copy of your driver's license and social security card or birth certificate (one of the last two are sufficient) for the form I-9. A copy of your passport alone is acceptable.
• A passport photo for our name badge (unless you come into the office). We have a digital camera.
• Two written references

If your present employer has competency data, I would like to have a copy for your file. Any questions, please contact our office at 317.787.8233.

Once we receive this information, we will make arrangements to complete the hiring procedure, answer any remaining questions, create your name badge and place you into a position. This process, although it sounds complex, is based on JCAHO requirements and will go rather quickly.
