We have enclosed our new employee packet for your review, completion and signature. Once this is completed, send it back to us immediately (or make arrangements to meet in person) with copies of:

• current resume
• negative drug screen
• copy of your ADA credentials
• certificate of completion or diploma from your program
• copy of an Indiana state licensure
• current PPD (TB) test
• copy of Hepatitis B vaccination or declination
• Copies of your MMR vaccines or titers
• current CPR and/or ACLS card
• copy of your driver's license and social security card or birth certificate (one of the last two are sufficient) for the form I-9.
• copy of your passport alone is acceptable.
• passport photo for our name badge (unless you come into the office)
• Two written references

If you have competency data, we will accept it. Any questions please contact our office.

Once we receive this information, we will make arrangements to complete the hiring procedure, answer any remaining questions, get you a name badge and place you into a position. This process, although it sounds complex, is based on JCAHO requirements and will go rather quickly.
